Agreement and eligibility
By using CareProof, you agree to these terms and the Privacy Policy. You must be at least 18 and authorized to enter and share information about each person represented in a care profile.
Purpose of the service
CareProof helps users organize caregiver-entered tasks, observations, routines, and summaries. It is not a medical device, healthcare provider, insurer, benefits administrator, emergency service, or law firm.
No medical, legal, or eligibility decision
CareProof does not diagnose conditions, recommend treatment, calculate an official benefit, or determine insurance, Medicaid, VA, waiver, or program eligibility. Reports are organizational aids and must be verified by the user and reviewed by the relevant professional or agency. For an emergency, call 911 or the appropriate local emergency number.
Your responsibilities
You are responsible for factual entries, obtaining necessary permissions, reviewing generated summaries, protecting account access, and correcting inaccurate information. Do not rely on CareProof as the only location for critical instructions or records needed during an emergency.
Acceptable use
Do not use CareProof to violate law, impersonate another person, access records without permission, submit malicious code, interfere with the service, scrape data, or make deceptive credential, provider, medical, or benefits claims.
User content and care-team access
You retain responsibility for information you enter. You grant CareProof the limited permission needed to host, process, display, and export that information for the service. Account owners control care-team access and are responsible for inviting the correct people with appropriate roles.
Availability and changes
We may improve, suspend, or discontinue features and may restrict access needed to protect users or the service. We aim for reliable operation but do not promise uninterrupted or error-free availability.
Disclaimers and responsibility
CareProof is provided on an “as available” basis to the extent permitted by law. You remain responsible for decisions made using the service. Nothing in these terms excludes rights or responsibilities that cannot legally be excluded.
Termination and governing rules
You may stop using CareProof and delete your account through the Privacy Center. We may suspend misuse or material violations. Applicable law governs these terms, subject to any mandatory consumer protections.
